The Wraparound program is a community based case management program designed to provide a wide variety of case management services as well as assistance in accessing a variety of other community services and support.
The Wraparound program provides services to children and adolescents who are not eligible for the CCS or other case management programs due to lack of Medical Assistance, or not being determined eligible. The wraparound program requires that a child/adolescent either be involved in, or is at risk of being involved in two systems of care (Mental Health and IEP Services, Youth Justice and Mental Health, etc.)
The focus of the Wraparound program is on empowering families to take an active role in their own recovery process and to involve informal supports in this process. Services are individual-centered and directed, which means the individual has a choice in the selection of those on their Recovery Team, what services they receive and who provides the services.
Services Include:
- Psychiatric symptom management and crisis prevention.
- Case management services to assist with accessing community resources.
- Assistance in learning and maintaining skills for achieving independence in a community setting.
- Education and skill development training services
